OC department and agency organization charts and information appearing in different reports and on the County web site are inconsistent and do not always reflect the Orange County government organization as it is operating.

Organization charts are not uniform or consistent from department to department nor are they signed, dated and approved by the CEO, BOS or the appropriate managers in authority, nor are they regularly maintained with an up-to-date status.
